Computer Frequently Asked Questions (FAQ) -- How To Request ITS Access for Partners, Contractors and Volunteers
 

After eAuthentication, employees needing access to ToolKit should contact Craig Ziegler in the Redmond Office, (541) 923-4358 Ext 110.

 

The following documents require Adobe Acrobat or MS Word.

Supervisors: 
1)  Please print and provide all of the following mandatory items to the new employee.
2)  Please include the graded tests with each packet you submit to the State Office.

SECTION A  --  Request for Computer Access

SECTION B  --  Background Investigation Packet
Item Format Notes
* SF 85, Questionnaire for Non-Sensitive Positions PDF, 336 KB Addendum Instructions for Completing SF-85 and SF-87
* SF 87, Fingerprint card (two cards required)   Available in Field Office

Note:  Contractor personnel ONLY must use the FD-258 fingerprint card (available by contacting Rita Ramos)
 
* OF-306, Declaration for Federal Employment PDF, 224 KB This is an Office of Personnel Management requirement, even non-Federal employees such as partners & contractors)
* OF-612 (Optional Application for Federal Employment) or may be substituted with a Resumé PDF, 224 KB This is an Office of Personnel Management requirement, even non-Federal employees such as partners & contractors)

If you substitute the OF-612 with your resumé (make sure to provide a physical location address for each employer within the last five years).

SECTION C  --  Training & Testing Material

* NRCS Oregon Computer User Security Agreement (Revised NRCS-IRM-02)  Word, 37 KB Supervisors -- Please retain the original copy in your files and submit a copy with packet.
